GENERAL ACCOUNTABILITY: The Community Health Worker´s primary function is to assist clients, as assigned: observation, monitoring and assistance with the tasks of daily living related to personal care, household management & provision of family support as applicable, in order to maintain or improve the client´s quality of life.We are currently hiring part time and full time CHWs, RCAs, PSWs to work in Lower Mainland, BC including Surrey, Vancouver, New Westminster, Langley, Maple Ridge, Pitt Meadows and surrounding areas. DETAILS OF POSITION Reporting to the Client Service Supervisor, the incumbent is responsible for: Perform assigned observation, monitoring, family support, personal care and housekeeping tasks according to individual competence and standards outlined by the provincial Ministry of Training, Colleges and Universities, as applicable Seek guidance from Client Service Supervisor or appropriate facility staff, as applicable, when unsure of how to proceed in a specific situation Refrain from performing tasks/procedures for which they have not received appropriate training (e.g. delegated procedures) Report changes in the client´s condition to the Client Service Supervisor or appropriate facility staff, as applicable Adapt to the individual client´s requirements and wishes while remaining within the parameters of the established care plan Document the service provided in the client care record while adhering to related policies, standards and guidelines Submit client reports according to procedure established in the local office Communicate with appropriate office personnel according to procedure established in the local office Respect client rights, including privacy and confidentiality of client information in accordance with applicable federal and provincial legislation (e.g. PIPEDA) In an emergency, access emergency response system and administer appropriate aid (e.g. First Aid, CPR) QUALIFICATIONS - Certificate from provincially recognized College for Community Health/Personal Support Worker and Health Care Programs required.
