We currently have the following opportunities at Swedish Medical Center:Position Summary:The Clinical Documentation Specialist facilitates modifications to clinical documentation through concurrent interaction with physicians and other members of the health care team to support that appropriate clinical severity is captured for the level of service rendered to all inpatients. The CDS will: Provide daily clinical evaluation of the medical record including physician and clinical documentation, lab results, diagnostic information and treatment plans. Be responsible for the day-to-day evaluation of documentation by the Medical Staff and healthcare team in accordance with the hospitals designated clinical documentation policies and procedures. Communicate with physicians, face to face or via clinical documentation inquiry forms, regarding missing, unclear or conflicting medical record documentation to clarify the information, obtain needed documentation, present opportunities, and educate for appropriate identification of severity of illness. Communicate with appropriate healthcare team members to ensure accurate and complete documentation is in the medical record. Demonstrate an understanding of complications, co-morbidities, severity of illness, risk of mortality, case mix, secondary diagnosis, impact of procedures on the final DRG, and an ability to impart this knowledge to physicians and other members of the healthcare team. Gather and analyze information pertinent to documentation findings and outcomes.Position Requirements:Licensure/Certification/Registration:Currently licensed as a Registered Nurse, CCS or Coding experience desirableEducation:Prefer BS in Nursing or Registered Nurse with ten years acute care experience or RHIA/RHIT with ten years inpatient coding experience. Advanced clinical expertise and extensive knowledge of complex disease processes with a broad clinical experience in an inpatient setting.Experience:Five years acute care hospital experience.Knowledge/Skills/Abilities:Clinical expert with at least 2 years recent inpatient experience; Excellent verbal and written communication skills; Proficiency in organization and planning; Proficiency in computer usage including database and spreadsheet analysis, presentation programs, word processing and Internet searching; Understanding of organizational policies and procedures; Working knowledge of quality improvement theory and practice; Ability and willingness to seek out and accept change; Demonstrates adaptability and self-motivation by staying abreast of CMS rules and regulations and incorporating those changes into daily practice; Knowledge of federal, state and private payer regulations.Denver, Colorado offers year-round outdoor recreation, as well as all of the amenities youd expect from a cosmopolitan city. Reviews electronic medical record documentation to obtain or verify diagnoses and procedures.Assigns accurate ICD-9 and CPT codes utilizing an electronic encoder application in accordance with hospital policy and regulatory body guidelines.Performs accurate, optimal DRG and APC assignment, in accordance with nationally established rules and guidelines based upon documentation within the medical record.Reviews electronic medical record documentation and abstracts statistical data into HIS abstracting system based upon hospital policy and regulatory body guidelines.Assists with quality assurance (peer) reviews to ensure data integrity and accuracy of coding, identifies opportunities for improvements, and makes recommendations for optimal enhancements.Assists with training of new coder, when needed.Assists Case Management and Patient Access Departments in providing appropriate CPT codes for preadmission and pre-certification requirements.Provides assistance to Clinical Documentation Management Program (CDMP) with appropriate DRG assignment, diagnoses and procedures sequencing, and coding and documentation training.Aggregates data from reviews and compiles reports for HIM MgmtCommunicates as necessary with physicians to obtain or clarify diagnoses and/or procedures via the internal physician query process and making sure the physician documents within the medical record.Maintains data integrity of coded and abstracted records within 95% accuracy.Verifies accuracy of transcribed reports/indexed documents and assigns errors to the appropriate queue in the electronic medical record application.Meets minimum coding productivity standards within 3 months of employment or maintains minimum coding productivity standards during employment while ensuring accuracy of coding. Represents the coding area in the hospital meeting/events when necessary.Performs other job related duties as assigned.Demonstrates components of ICARE value statement.Demonstrates components of ServicePRIDE standards.Follow all safety rules while on the job. Experience Certified Coding Specialist (CCS) required.Registered Health Information Technology (RHIT) required with 10+ years inpatient coding experience in an acute-care setting with minimum 2 years coding experience in a 200+ bed, acute-care facility. OrRegistered Health Information Administrator (RHIA) required with 7+ years inpatient coding experience in an acute-care setting with minimum 2 years coding experience in a 200+ bed, acute-care facility.

Program Lead Opportunity!

To serve as the primary liaison with community physicians in marketing and recruitment in the uptake and successful implementation of Electronic Medical Record (EMR) systems in their offices and participation in regional pediatric data sharing and quality initiatives as part of the Children�s IQ Network. Coordinates status reports, as well as participating on teams and working with Children�s IQ Network leadership on this initiative. Assigns work to team members to assure timely completion of Network milestones and deliverables as appropriate.Physician or Nurse Practitioner. Formal informatics training preferred. Business or marketing skills desired. Electronic Medical Record and Pediatric Primary Care Experience required.
