Key Responsibilities1. Abstracts, sequences and assigns diagnosis and procedure codes according to ICD-9-CM coding conventions and Uniform Hospital Discharge Data Set (UHDDS) definitions. Assigns CPT-4 procedure codes as necessary and analyzes and assigns diagnosis groups (DRG) utilizing specialized computer software. Enters data into computerized abstracting system.2. Follows-up on outstanding un-coded and incomplete charts. Performs periodic quality control audits of work. Maintains required volume/productivity statistics.3. Responds to outpatient clinics requests for ICD-9-CM diagnosis codes via telephone hotline.4. Develops and maintains professional skills and knowledge through attendance at inservice programs, conferences, workshops and other educational programs and review of current literature. Assist in training new personnel in coding procedures. 5. Maintains neatness and organization of work area. Routes misdirected and/or loose medical record components to appropriate department/area and returns completed medical records to file area. 6. Contributes to teamwork and performs other duties as required by the position. 7. Performs other related duties as assigned by supervisor or manager.Minimum Knowledge and Skills required by the Job 1. Work requires the level of knowledge typically acquired through completion of two years of occupationally specific education or an Associate´s degree in Medical Records. This includes but is not limited to: medical terminology, anatomy, physiology, pathophysiology.2. Minimum of 3 years hospital based coding experience3. Work requires in-depth knowledge of medical records coding utilizing ICD-9-CM and CPT-4 Coding conventions. Work requires knowledge of the various DRG systems (CMS DRGs, AP-DRG, and APR-DRGs).4. Working knowledge of health records, computer systems, Microsoft applications, data integrity, and processing techniques required. 5. Achieve a minimum 80% accuracy rate on the pre-employment coding assessment.6. Must be detail oriented, ability to remain focused, have good organizational, interpersonal and communication skills.7. Work requires ability to solve problems appropriately using job knowledge and current policies/procedures. Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical, technical principles or clinical knowledge. 8. Work requires knowledge of the HIPAA rules and regulations pertaining to patient´s information and confidentiality. 9. Effective communication, analytical ability and great follow-up skills are required to accurately perform duties. 10. Demonstrate ability to prioritize task base on section needs.11. Must have the ability to adapt to ongoing changes in work environment.
