Performs complex technical and related duties to analyze, review, abstract, code, and conduct risk surveillance on medical information from a variety of hospital data sources MINIMUM QUALIFICATIONS: 1. Associate’s degree in Health Information Technology or equivalent combination of education and experience may substitute 2. Certification as a Registered Health Information Administrator (RHIA), Certified Coding Specialist (CCS) or Certified Procedural Coder (CPC), including one year experience ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Assigns diagnosis and procedure codes by reviewing health records/information to ascertain all appropriate diagnosis and/or procedures for ICD-9-CM and CPT-4 designation 2. Determines the content and validity of physician written orders to ensure an accurate diagnosis is in accordance with coding guidelines 3. Abstracts and enters clinical data from the medical record into RPMS 4. Concurrently reviews and interprets medical record documentation in both hard copy and computerized formats to determine the diagnoses and procedures for specified visits 5. Corresponds with medical staff and other providers regarding records with omissions and/or inconsistencies to ensure timely completion for proper quantitative analysis 6. Determines principal diagnosis and procedure and properly sequences all secondary diagnoses 7. Abstracts data for benchmarking, quality assessment or other studies. 8. Receives and responds to requests for release of confidential medical records information; verifies confirmation of release authorization 9. Participates in departmental orientation, on the job training and quality assurance programs/initiatives 10.
