Job Description - External:The purpose of this position is to provide leadership and direction for implementing a System driven Planned Care Initiative at the PeaceHealth Medical Group Regional level. This person coordinates System Planned Care work with Regional work to ensure that System clinical and operational goals related to the Planned Care initiative are met.NATURE AND SCOPE:The Planned Care Program Manager reports to the System Director, Planned Care. The position has a matrix dyad relationship with the Primary Care Regional Clinic Director/Manager. This position leads and directs the system planned care work, coordinating and aligning any associated regional work as needed to achieve the Planned Care Vision. The individual participates in strategic decisions specific to the Planned Care initiatives. The position requires both strategic and tactical implementation skills and requires solid program planning and implementation skills. Clinical and operational knowledge of healthcare delivery systems and finely honed facilitation and communication skills are essential. This person is expected to be an accurate observer and interpreter of issues involving Planned Care implementation in the regions and must be able to constructively bring these issues to the Regional PHMG leadership team and to the System. On a day to day basis, the individual is responsible for artfully blending analysis, clinical skills and knowledge, and leadership to develop realistic project approaches and plans. The scope of performance includes the ability to effectively communicate PeaceHealth Vision 2012 and strategic plan and the ability to provide staff with the leadership, coaching and support required for achieving cultural change. All activities are expected to be implemented in a way that supports staff engagement and organizational learning. The incumbent will maintain knowledge regarding current programs being implemented in primary care throughout the country and be able to share and articulate those ideas with colleagues at both the regional and system level. DUTIES AND RESPONSIBILITIES:-Support PeaceHealth Strategies by designing, managing and coordinating agreed-upon system projects, activities, and assignments at the regional level. -Collaborate with System Planned Care Director and with the Regional Clinic Director/Manager to:-Implement initiatives and projects related to Planned Care. (Examples include: implementing disease specific evidence based guidelines, re-designing office processes to support guidelines, team development, patient centered care, and coordinating grants evolving from partnerships with payers etc.) -Lead the Regional Planned Care Leadership Team and coordinate regional work teams as required -Develop regional annual work plan with timelines and milestones-Facilitate development of cross regional standardized policies, practices, protocols and procedures where required to achieve planned care goals. -Establish internal regional processes for evaluating progress-Define measurable goals and objectives -Assist in the identification of staff and team training needs and in the development of plans for improving individual and team effectiveness -Coordinate and escalate risk issues and progress barriers to Regional Planned Care Leadership Team and System Planned Care Director and others as required. -Prioritize and manage tasks so that goals are achieved while at the same time honoring relationships and PeaceHealth values. -Develop effective relationships within the regional medical groups as required to achieve project goals. -Coordinate communication plans to ensure alignment between regions and system; serves as communication conduit between all regional and system key stakeholders.-Prepare leadership reports relating to projects, including status on projects and goals. -Work collaboratively with Analytics, Web Services, CHR and other HID resources.-Support and align all work with PH Change Leadership Strategy.-Establish proficiency with and keep abreast of national trends in assigned project areas.-Participate in cross-regional teams and committees, as assigned. QUALIFICATIONS:Education Bachelors degree required, Masters Degree or advanced training preferred. Clinical Degree/license preferred.Experience Five years experience in a healthcare setting. Progressively responsible experience will include project management, electronic medical records, group or project facilitation, as well as demonstrated successful implementation of clinical programs or quality improvement projects. A working knowledge of the nature and impact of key organizations (JCAHO, CMS, IHI) on PeaceHealth quality measurement and reporting requirements. Strong proficiency with MS Word, Excel, and other standard PC applications is required. Demonstrated experience working successfully with physicians and other clinicians is preferred. Must be able to work efficiently, independently and responsibly with minimal supervision and short time frames. Must also be able to work as part of a team, including at times in a virtual team environment. Demonstrated ability to work with a broad array of people from diverse backgrounds and from various levels of management, ranging from line analysts to Chief Executives, Physicians and other Senior Health Care Leaders, including quality leadership across PeaceHealth. Must be adaptable and flexible with the ability to initiate and support change. Must be able to work with the ambiguity inherent with innovation and design.Skills Strong project management skills and excellent organizational and communication skills, both oral and written, are required. Ability to facilitate discussion, coach, and train concepts related to team process. Must be willing to learn a variety of healthcare improvement tools and methodologies. Ability to cope with multiple demands and extremely precise, detailed and confidential work. Ability to prioritize work effectively, self-motivated and able to manage multiple tasks simultaneously.
