Job Description Position Summary: Assures that assigned patient records are scanned and indexed properly so that the information is easily identifiable and available to the end users, according to legal and regulatory requirements. Responsibilities Essential Duties & Responsibilities including but not limited to: 1. Verifies and matches each batch number to patient medical record 2. Reviews rescanning queue using indexing software 3. Identifies any Index exceptions on flatbed scanner that need to be rescanned or improved in quality and rescans as needed. 4. Performs a random sampling of patient data in Documentum. Verifies all patient information against paper medical record and corrects as needed. 5. Verifies that all form types are batched appropriately and correct if necessary 6. Confirms scanned images are reconciled with paper Medical Record. 7. Maintains an accurate backup system in order to carry out uninterrupted service. 8. Removes placer sheets and reassembles medical record if necessary. 9. Charges medical record and returns to Central Files 10. Supports and assists colleagues with related electronic systems within area of responsibility. 11. Acts as liaison between other Lahey departments providing, instruction, information and communication to appropriate superior. 12. Identifies and indexes documents into appropriate electronic folders. 13. Reviews electronic images within the batch to confirm image quality, order and appropriate rotation. Determines if images need to be enhanced for clarity or to delete, add or revise episodes. 14. Prepares and scans batches of documents into the electronic document management system as needed. 15. Works closely with supervisory staff to insure customer satisfaction as related to imaging duties 16. Assists end users in resolving issues related to data retrieval and other indexing issues 17. Follows through to satisfactory resolution of all QC/indexing outliers 18. Incorporates Lahey Clinic Guiding Principles , Mission Statement and Goals into daily activities. 19. Complies with all Lahey Clinic Policies. 20. Complies with behavioral expectations of the department and Lahey Clinic. 21. Maintains courteous and effective interactions with colleagues and patients. 22. Demonstrates an understanding of the job description, performance expectations, and competency assessment. 23. Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards. Qualifications Minimum Qualifications: Education: High school diploma or equivalent. Ability to access and process electronic information utilizing computer technology. Experience: Minimum of 1 year of experience performing administrative duties involving document processing utilizing computer technology.

Reviews records of inpatients to ensure the accurate selection of the principal diagnosis and procedure and any complicating or preexisting conditions, including present on admission diagnosis. Reviews facility case mix for financial data to identify trends and problem areas. Directly works with physicians and case managers to establish concise physician documentation. Involved in physician awareness and education of documentation guidelines in relation to coding guidelines that support appropriate reimbursement.
