Position Summary Purpose: To assist patients and their families in making psychosocial adaptations specific to their diagnosis, while facilitating the optimal utilization of health care services and community resources in accordance to the age and developmental level of the patient. Knowledge and Skills: Required for completely satisfactory performance in this job is knowledge of medical social work, psychosocial models and theories and community resources. Also required is the ability to conduct a thorough psychosocial assessment and develop, implement and evaluate a plan of care. Ability to conceptualize the psychosocial needs presented by the assigned patient population, to intervene with patient/family behavior, to coordinate community resources and to handle crisis and complex psychosocial situations. Ability to handle grief. Ability to demonstrate initiative, good judgment and problem solving skills. Ability to demonstrate excellent professional listening and communication skills. Formal Education, License, and Experience: The formal education normally associated with completely satisfactory performance in this job is a Master´s degree in Social Work from an accredited School of Social Work. Licensure as a Social Worker in the State of Texas is required. Previous employment in health care, a health care specialization in schooling or a field work practicum in health care preferred.
