* MULTIPLE POSITIONS AVAILABLE *Job Summary:The Clinical Social Worker is a graduate level professional that has demonstrated expertise in the assessment and treatment of patients in a variety of inpatient and outpatient practice settings. The Clinical Social Worker possesses clinical leadership abilities and is knowledgeable and skillful in communication and the treatment process, and provides services to the pediatric, adolescent, adult, geriatric or dual diagnosed patients/families/staff in the Yale New Haven Hospital network. The scope of practice includes but is not limited to diagnostic assessment, coordination of care, counseling, psychotherapy brief treatment, behavior modification and mental health consultation, and disposition planning. Clinical Social worker will report to the Manager and work collaboratively with members of the health care team.Job Requirements:Master´s Degree from an accredited school of social work. Qualifying social work experience consists of a minimum of 3,000 hours of post-masters social work. Licensed Clinical Social Worker (LCSW) from the state of CT, or those holding licenses from other states, will need an endorsement and meet the State of Connecticut license requirements. Must also have the ability to perform complex bio-psychosocial assessment and case formulations, including family assessments, and to execute timely and effective interventions, in collaboration with the team. This requires excellent analytical skills to evaluate care needs and gather relevant clinical data as well as excellent interpersonal communication and leadership abilities.Possess skills to provide expert verbal and written clinical presentation and consultation along the continuum of care and also must possess the ability to work collaboratively, effectively, and efficiently with collateral health and mental health care professionals to ensure seamless treatment planning and execution.Possess the experience and knowledge base required to provide clinical supervision to colleagues pending licensure, and participate in teaching within the hospital, and university setting.Possess the demonstrated ability to manage complex clinical care, assume appropriate decision making responsibility and collaborate in development and implementation of practice standards to achieve the highest quality and most appropriate patient care. The Clinical Social Worker requires advanced and creative planning, problem solving, leadership, and decision making skills. The Clinical Social Worker will participate in Quality Improvement activities and departmental/hospital committees.Master´s Degree in Social Work
