It is the largest nonprofit provider of senior health care and housing in New England. Our health-care network comprises Hebrew Rehabilitation Center, a long-term, sub-acute, and long-term medical acute care facility in Boston; three senior housing sites-Jack Satter House in Revere, Simon C. Fireman Community in Randolph, and Center Communities of Brookline; and Orchard Cove, a life-care community in Canton. The Institute for Aging Research in Boston, affiliated with Harvard Medical School, is one of the most prestigious aging research facilities in the country. HSLs NewBridge on the Charles, our premier multigenerational continuing care retirement community in Dedham, consists of 256 independent living units, 50 assisted living units, 41 memory support assisted living units, and a 268-bed Health Care Center, which offers long-term and sub-acute care. Hebrew SeniorLife also offers services to the community, including Adult Day Health programs and outpatient clinics in audiology, exercise, memory disorders, osteoporosis screening, and rehabilitation.Reporting to the VP of Nursing and Patient & Family Centered Care of Hebrew Rehabilitation Center (HRC), the Director of Patient & Family Centered Care Services is responsible for the implementation of all patient and family centered care principles and concepts in the delivery of patient care at Hebrew Rehabilitation Center. The position will work in collaboration with all disciplines in the development and integration of patient and family centered care concepts and principles in clinical pathways and quality initiatives. Develop effective strategies for recruiting patients and family members and sustaining a positive momentum to Patient and Family Centered Care. Develop partnerships and affiliations with various providers and organizations to optimize and promote patient and family centered care. Provide a structure for ongoing program quality monitoring and enhancement. Assist staff and management across all disciplines in understanding the roles of patients and families at meetings or on committees and task forces. Identify opportunities to include patients and families in policy and program planning, implementation and evaluation.
