Interprets evaluation findings to determine the nature and extent of dysfunction. Identifies precautions/contraindications to speech therapy treatment. For those patients not appropriate to speech therapy management, communicates evaluation findings and/or makes recommendations to primary medical practitioner regarding patient disposition. For those patients appropriate to speech therapy management, identifies problems through integration of evaluation techniques/tests. Establishes objective and measurable treatment goals with time frames. Plans treatment to achieve established goals. Initiates discharge planning for speech therapy. Implements the treatment program through direct treatment of the patient, with agreement of the patient and/or family members and coordinates treatment program with other health care professionals. Provides advice and education to patient and family regarding participation in the treatment program, extension of the program into the hope situation and identifies precautions regarding high-risk activities. Regularly reassess clinical sign and symptoms to determine effectiveness of treatment, progress of patient toward goals and the need for modifying goals and treatment. Provides care for appropriate age/developmental level to adults, children, and infants. Devises forms and written instructions as needed. Initiation of treatment in acute setting with possible follows through in the medical clinic or referral to the home setting. Discharges patient from Speech Therapy when the patient has received maximum benefit from Speech Therapy management in the opinion of the therapist and/or medical staff. Provides accurate, timely documentation of speech therapy management. Demonstrates a high level of professional conducted commensurate with standards of the American Speech and Hearing Association and conforms to the canons of the established code of ethics.
