We are located in Beverly Hills close to the heart of Los Angeles.Summary of Essential Job Duties:The role of the social worker in our Cardiac Transplant is to assess and identify the psychosocial barriers to achieving maximum adjustment and functioning relevant to transplantation. The Transplantation Social Worker has the professional training and experience to deal with the psychosocial issues Referral and Case Finding: Upon notification by the Heart and/or VAD Transplantation Team, the social worker meets all new potential heart and/or lung transplantation patients and families; services will that will be provided include: family assessment, crisis intervention, supportive counseling, placement assistance, and discharge planning to assist transition into the community.The social worker participates in in/out-patient rounds, attends patient related conferences and meetings as appropriate, is involved in staff teaching/education, serves as an interagency liaison, assists the Heart and/VAD Transplantation Team in its planning, implementation, and monitoring of new projects and/or programs; reviews transplantation social service provisions and makes recommendations when needed; serves as a patient advocate; and monitors changes in governmental and/or third party payer regulations, programs, and/or policies.Physical Demands:Patient care, driving, lifting, and bending Education and Work Experience Preferences: LCSW is required
