APD is known in the community for providing the best in personal care in a family and community attitude, where everyone has a great story to tell about their experience.Essential Position Functions:* Responsible for the evaluation of all patients referred for Physical Therapy.* Responsible for the completion and submission of all reports pertaining to the evaluation.* Sees that all referral sources are advised of department action on the case referred.* Practices Physical Therapy consistent with the American Physical Therapy Association Code of Ethics and Standards of Practice.* Maintains a patient treatment schedule consistent with the needs of the department and patient.* Confers with the persons most closely associated with the management of the patient and keeps them informed of any additional patient needs, treatment modification and /or progress.* Responsible for the preparation and submitting of department reports, ensuring confidentiality of all records, including: Evaluation reports/summaries; Discharge reports/summaries; Progress reports to referring physicians and agencies; Current progress notes on all patients within the assigned area; Special reports as requested by the director or supervisor; and Daily attendance records and summaries of same.* Keeps abreast of new techniques and trends in the field of Physical Therapy.* Attends conventions, meetings or special courses whenever possible and shares with the department personnel information gained thereby.* Prepares and gives talks to community groups who have made requests for same.* Maintains good public relations with all community sources.* Participate in planning and implementing new programs in the Physical Therapy Department.* Care for the physical area and equipment related to Physical Therapy Department and maintain highest possible level of departmental cleanliness and safety.Position Standards:* Graduate of a school or Physical Therapy School approved by the American Physical Therapy Association. Currently licensed or eligible for licensure in New Hampshire. No previous experience necessary.* Must have thorough knowledge of principles, literature, standard practices, methods and new developments in the area of physical medicine.
