TITLE (REQ ID): PHYSICAL THERAPIST I (83547) DEPARTMENT: Physical Therapist EMPLOYMENT TYPE: FULL TIME SHIFT: DAY HOURS PER PAY PERIOD: 80 SHIFT HOURS: 8HOUR FACILITY: DOMINICAN HOSPITAL SANTA CRUZ LOCATION: SANTA CRUZ, CA FACILITY INFORMATION POSITION REQUIREMENTS The physical therapist reviews patients’ biographical, medical, social, and functional history, evaluates all aspects of functional mobility skills of patients, establishes realistic functional goals, and develops a treatment plan to attain these goals for each patient.They administer effective therapeutic procedures to improve, restore, and develop function for each patient and delegates appropriate tasks to rehab techs and/or student interns. They provide comprehensive instruction to family/attendant in safe and appropriate techniques to compensate for dysfunction and minimize further disability. They select and instruct in the use of orthotic devices. They maintain accurate and timely medical records including evaluations, progress notes and daily treatment records, and discharge summaries. They communicate in an effective and timely manner with all associates, patients, family members, case reviewers, and physicians. They attend and effectively participate in all appropriate meetings, and demonstrate the ability to participate as an effective member of a rehabilitation team. They maintain and enforce safety standards established by the hospital for patients, visitors, and associates, maintain acceptable personal and professional appearance, and maintain ethical standards established by APTA.Education: - Graduate of an APTA approved school of Physical Therapy. Special Training: (including license): - Current and valid physical therapy license, according the laws of the State of California. - CCS paneling for pediatric car- Current BLS certification requiredExperience: (minimum, plus desired): - Minimum of one year of experience as a physical therapist in physical disabilities setting, preferably in acute care or acute rehabilitation. - Knowledge of physical therapy theory, treatment, and evaluation in the areas of neurology is recommended.
