Formulates a discharge plan in a timely manner that matches patient with services best able to meet the patients needs. Provides ongoing assessment during hospitalization to ensure appropriate referrals based on status at the time of discharge.Demonstrates the ability to effectively communicate the recommendations of the treatment team to the patient, the patients family, caregivers and other support systems.Effectively assesses the psycho-educational needs of the patient, family, caregivers and support systems.Demonstrates the ability to increase the awareness of existing community resources to the patient, family, and caregivers.Effectively performs in a leadership role as a treatment team member in the treatment team process.Provides appropriate treatment based on the psychosocial assessments and the recommendations of the treatment team. Performs individual and family counseling, provides psycho-educational resources and facilitates group counseling.Consistently conducts a psychosocial assessment within 72 hours of admission.Counsels patients and their families regarding psychosocial needs or problems related to their mental heath treatments. This includes concerns regarding housing, insurance, medications compliance and financial issues.Provides psycho-educational materials and resources to the patients, families, and other caregivers.Demonstrates the ability to effectively advocate for the patient.Provides family and individual therapy based on recognized therapeutic techniques. Gives and receives constructive feedback appropriately.Embraces the concept that as professionals in the complex and ever changing field of mental health treatment Behavioral social workers must be active life-long learners. Accepts the added responsibilities of being a master/licensed level professional in terms of educational demands. Demonstrates advance practice in assessment and clinical skills and ability to plan and facilitate therapy groups.Qualifications:Masters in Social Work with LCSW licensure required.Two years of social work experience required.
