We offer both short and long term care meeting the diverse needs of the community with 24 hour skilled nursing care, individualized treatment plans and active programs of restorative care. Were able to enhance the quality of life of our patients by providing a variety of activity programs like community based recreational, social and educational activities and comfortably furnished lounges and patios for socializing and relaxation. Our full complement of restorative services will ultimately help individuals regain and maintain activities of daily living and mobility at their highest achievable level. Our focus remains in assisting individuals toward their goal of independence. The individualized restorative programs include physical therapy, occupational therapy and speech-language pathology. For the residents that require continuous intensive level of care, we provide them with a sub-acute unit. The intensive medical and skilled nursing care residents receive prepares them for re-entry or discharge to the next appropriate level of care. Patients are also provided with a dedicated unit of care for Alzheimers and related dementia residents. In addition, our staff is particularly sensitive to the needs of residents and families during the later part of ones illness. This is something we don´t take lightly. Our staff is well trained in dealing with end-of-life issues and the focus has been on providing palliative/pain management care for over 12 years in conjunction with local hospice agencies. The therapist participates as an integral part of the interdisciplinary team, manages resources to meet the needs of the patient/resident and related customers balancing outcomes and cost effective and efficient treatment.Reporting to:* Director of RehabilitationSupervision of:* Supervise clinically all assistants and aides of patients to which the therapist is directing or overseeing the delivery of services according to an established plan of care. Essential Duties and Responsibilities:* Provide and direct patient/resident rehabilitation care within all general accepted practice methods, meeting all facility, state, federal, and accrediting standards. * Evaluate patients/resident needs and establish effective and efficient treatment plans for the patient/residents assigned. * Manage and coordinate the supervision of treatment plans with all staff and other disciplines as indicated. * Participate in the screening of all patients according to directed schedules and practices. * Provide feedback o the Team Leader on all clinical and rehabilitation issues. * Participate in all facility directed processes and services. * Show effective management of treatment and non-treatment related activities. * Perform necessary clinical documentation to meet facility, state, federal, and accrediting agency requirements. * Maintain complete, accurate and timely billing records to meet facility, state, federal, and accrediting agency requirements. * Assist with marketing efforts as directed. * Participate in in-service training programs as requested. * May travel to other facilities to provide services. * Perform other duties as required.Education: * Graduate of an accredited college or university with minimally a bachelors degree in physical therapy. * Foreign trained therapists must meet the education standards as defined by the state licensing board in the state they practice. Professional Experience: * Prefer experience/knowledge of managing other rehabilitation personnel. * Prefer experience/knowledge in long term care facilities. * Prefer experience/knowledge of managing cost and productivity. * Prefer Outpatient experience/knowledge. * Knowledge in governmental and regulatory guidelines in designated healthcare settings. Certification/Licensure: * Actively licensed therapist in assigned state(s) of employment as a Physical Therapist or related health care setting.
