Collaborates with treating physician, psychiatric and allied health professional team to plan and direct each individual members treatment program. DUTIES: Assesses, develops and coordinates a clinical treatment program for the diagnosis, treatment, and referral of Health Plan members with acute or chronic psychiatric disorders. Participates in staff conferences to select, plan, and evaluate treatment programs. Consults with medical, nursing and psychiatric staff regarding diagnosis, strengths and deficits. Provides outpatient psychotherapy to individuals and groups. Charts members treatments and progress in accordance with state regulations and department procedures. Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, including adherence to prescribed medication regimens within the scope of practice. Prepares reports, intakes, treatment plans, case summaries and maintains ongoing confidential records. Interprets psychiatric treatment to members family and helps to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment. Refers members family to community support services and resources. Utilizes resources of public and private agencies and community organizations to meet the needs of members. Collaborates with physicians in screening and evaluating patents for psychotropic medications. May supervise PSW Assistants and PSW Interns. Provides case management, including the development of relationships with community resources and services in order to make appropriate referrals. Develops, implements, and evaluates behavioral medicine program. Provides psycho-educational classes and groups. This position may be required to work on-call hours at the discretion of management. These evaluations include a risk assessment, mental status examination, and a psychosocial history that leads to appropriate dispositions for patients. This includes gaining 5150 privileging in both San Joaquin and St. Anislaus counties. This also includes successful credentialing at the various valley hospitals. Experience competency in responding to physicians´ consultation requests by providing on-site assessment, treatment planning services, and crisis intervention. Assessment skills applicable to determining appropriate follow-up mental health services, including need for involuntary psychiatric hospitalization. Able to conduct DSM-focused assessment and diagnoses, crisis intervention/resolution, and brief individual and/or family interventions. Excellent interpersonal and communication skills working in an interdisciplinary medical center environment coordinate patient care with the psychiatric call center as well as with ED and hospital nursing and ancillary staff. Provide consultation services for ED and HBS physicians regarding patients´ progress and treatment; confer with attending physicians regarding hospitalized patients and recommend treatment plansThis position will be covering Emergency Rooms at area hospitals within the Modesto Area in 2009then responding to emergency rooms in the Central Valley area starting sometime in 2010. This person must be willing to travel to these various sites as needed, be available for flexible hours and shifts depending on need including weekend work.
