Assess physical and psychosocial status and home environment. Develops,implements and documents, according to State and Federal regulations, implement the plan oftreatment for patients. Communicates with physicians concerning treatment plan and course.Supervises Home Health Aides. Attends and participates in team meetings, in services and committees. Completes documentation in a timely manner. Evaluates patients and interprets evaluation findings todetermine the nature and extent of dysfunction. Establishes treatment goals and plans treatment toachieve established goals. Initiates discharge planning for physical therapy.
