Must be willing to work all shifts, a minimum of 6 shifts per month including evenings and overnights.Provides assessment, treatment, resource information and referral, education, utilization review, and disposition planning to individuals, families and significant others, and groups. Functions as part of an interdisciplinary team which can include physicians/psychiatrists, case managers, therapists, and other medical providers to develop individualized treatment plans, provide treatment, case management and disposition. Services are provided using accepted psychotherapeutic models and interventions, including integrative healthcare options appropriate to the patient´s diagnosis.Participates in initiatives and programs related to the vision, mission, and values of PVHS. Understands and represents the strategic goals of service lines as assigned. Develops and maintains knowledge and understanding of medical and/or psychiatric disease processes, treatments, and emotional issues related to these diagnoses. Coordinates psychosocial care and interfaces with third parties and community agencies throughout the continuum of care. Demonstrates proficiency in customer relations with clients and community systems. Initiates 27-10 holds as appropriate to patients situations.Masters Degree in Social Work, Counseling, Psychology or a related field. Previous LCSW, LPC, or LMFT experience in hospital preferred. One year LCSW, LPC, or LMFT experience preferred Licensed Clinical Social Worker in the State of Colorado, Licensed Professional Counselor in the State of Colorado, Licensed Marriage and Family Counselor in the State of Colorado, or LSW with advancement to LCSW within identified Board of Social Work timeframe. Maintains membership in professional organizations or acquires specialized certifications and trainings if preferred or required.
