Brack Trauma Services Responsibilities 1. Manages Social Work, Eligibility, and PPAP staff in regards to daily operations and performance evaluation to achieve goals and ensure compliance with regulatory guidelines. 2. Maintains a small caseload and provides coverage for social service staff as needed. 3. Develops, implements and provides oversight to departmental performance improvement activities as well as collaborating with medical staff leadership, nursing leadership, and administrative leadership to improve performance and processes within the network clinic system. 4. Provides administrative oversight for provision of Social Service Responsibilities in the Network Community Health Clinics including managing the social service budget and establishing program standards. 5. Provides oversight, manages, tracks and reports on social service productivity. 6. Provides clinical supervision and finds resources for counseling/therapeutic services provided by the clinic social workers. 7. Develops, maintains, and implements policy and procedures for Social Work, Eligibility, and PPAP direct reports. 8. Networks with community and Seton Network resources to increase awareness of the Seton Community Health Clinics and to facilitate access to these services for the clinic patient population. 9. Participates in ongoing short and long range planning to help ensure the organizations social service needs are met and to maximize revenue and savings afforded to the network through therapeutic social work services and through savings generated by the PPAP staff. 10. Coordinates the selection, development and management of personnel. Minimum Qualifications Four (4) years of social work experience is required, to include experience providing individual or family counseling, and one (1) year of experience in a leadership role. Licensed Master or Clinical Social Worker (LMSW / LCSW) in the State of Texas is required. DCMC Case Management Responsibilities 1. Provide assessment, social services and counseling to patients, their families and staff in relation to social, psychological, financial and family situations including discharge planning as needed. 2. Interprets and communicates necessary information regarding patients and families including social history information to staff and physicians. Coordinates planning, makes recommendations and facilitates decision-making concerning individual patient care plans especially regarding the home situation. 3. Initiates and conducts educational programs for all levels of staff relating to issues such as alcoholism, coping with disability, death and dying, self-esteem and child abuse. Develops departmental projects in specialty areas such as Neonatal unit, Stroke Team, Orthopedics, Day Care. 4. Develops and maintains good working relationships with outside community health and social agencies and reaches out to the community to develop new resources for meeting patient needs. Conducts training programs with outside organizations regarding health and wellness. 5. Makes home visits to patients when necessary for adequate follow-up. Participates in on-call and weekend rotations for the department. Minimum Qualifications License to practice Social Work in the State of Texas (LMSW) must be obtained by the end of the first six months employment. Preferred Qualifications This LMSW position is for the Neuro/Psych unit at DCMCCT. Experience with the pediatric neurological and /or psychiatric patients is preferred. Skills Must have skills in interpersonal communication and problem assessment including patient, empathy and diplomacy. Residents assume clinical responsibilities according to their year of training as directed by each program. Responsibilities include: - Participating in providing safe, compassionate and ethical care. - Developing an understanding of how to provide cost-efficient care in an environment that seeks to minimize errors, while delivering evidence-based care. - Developing an understanding of the medicolegal and ethical principles of care. - Understanding the socio-economic aspects of medicine and the delivery of care within the various systems of care. - Understanding the role of the resident within the health care team. - Participating in the various committees and councils, if invited or elected, to assure effective communication, improve care, and monitor the care provided to the patients within the facility. - Participating in research programs and educational presentations, as required by the individual programs. - Adhering to the guidelines and regulations of the medical staff as well as the policies of the graduate medical education program. 2. Assumes progressive clinical responsibility as training progresses. 3. Evaluates the programs effectiveness in teaching and in providing the necessary supervision. 4.
