Physician Assistant I: (Primary Care) Skilled Nursing Facility/Long Term CareLake Scranton Area125 Scranton/Pocono HwyScranton, PA 18505(Will cover two to three nursing home facilities)
This nursing home/skilled nursing facility based advanced provider works in conjunction with Community Practice, Geisinger Health Plan (GHP) case management staff, and skilled nursing facilities to ensure GHP members are appropriately managed in the sub-acute an/or long-term care setting. The Physician Assistant provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting. Essential functions include, but are not limited to the following: - Responsible for the treatment of members in the nursing home and skilled nursing facility settings. Performs comprehensive SNF assessment to include psychosocial factors (when applicable) identifying the anticipated short-term and long-term needs of the member.- Identifies and refers members into appropriate disease and case management programs. Identifies the need for hospice care as appropriate in the terminal period of chronic diseases. - Refers appropriate members to the GHP case manager for hospice program election or to be designated as a Potential Hospice candidate and monitored for decline. - Must be able to take weeknight and weekend call as needed on all assigned members. - Must be able to travel to various nursing home locations as assigned. This provider collaborates with the CPSL physician for all patient related responsibilities and deals directly with CPSL clinical staff, Geisinger Health Plan (GHP) case management staff and skilled nursing facility staff members to coordinate patient care requirements. The candidate must be certified to work with adult population age group. Must be able to travel to various nursing homes locations (possibly one in the morning and one in the afternoon.). Must be able to take weeknight and weekend call as needed Previous nurse practitioner in a skilled nursing facility or with geriatric patients is strongly preferred.The successful Physician Assistant candidate will require completion of an ARC-PA approved physician assistant program, NCCPA certification or eligibility through Pennsylvania State Board of Medicine licensure. Must have current BLS and have or be eligible for ACLS. Must be able to travel to various nursing homes locations (possibly one in the morning and one in the afternoon.) Must be able to take weeknight and weekend call as needed Previous physician assistant experience in a skilled nursing facility or with geriatric patients is strongly preferred.Our successful candidate will have a strong desire to work with patients and their families. Additionally, the candidate should have prior advanced provider and/or skilled nursing facility experience. The candidate should have excellent people skills, demonstrated concern for the health and well-being of others, superior written and oral communication skills, computer literacy including electronic medical record documentation and the ability to work both as a team member and independently.
