JOB SUMMARY: Provides appropriate exercise instruction and motivation to patients in accordance with specific needs, abilities and medical condition MINIMUM QUALIFICATIONS: 1. Completion of a course of study leading to a high school diploma or equivalent 2. Current and valid BLS certification 3. Must have a current and valid Arizona drivers license ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Provides exercise instruction and motivational support to patients regarding proper and safe use of exercise equipment relative to individual exercise program needs 2. Supervises exercise programs initiated by Exercise Specialist and/or Physical Therapist for groups and/or individuals with various diseases states including, but not limited to: diabetes, cardiovascular compromise and musculoskeletal disorders 3. Initiates and monitors exercise programs for participants with less complex medical histories 4. Maintains equipment preventive maintenance and cleaning logs 5. Assists patients to register in computerized system; monitors attendance and participation to ensure accuracy of data 6. Performs cleaning, stocking and general care of gym areas, exercise machines and equipment 7. Monitors equipment set-up and instructions to patients to ensure ongoing safety and proper use of equipment 8. Performs assessment of participants tolerance to exercise program and personal motivation for continued participation 9. Monitors patients vital signs throughout exercise regimen; refers medical issues/concerns to Physical Therapist and/or technical issues/concerns regarding disease management during exercise to Exercise Physiologist 10. Recognizes signs and symptoms associated with cardiac distress; initiates CPR procedures and/or other emergency first aid; seeks appropriate medical assistance to ensure safety of patient 11. Provides outreach services to community members by presenting program information in schools, district meetings, health fairs and senior centers 12. Participates in departmental orientation, on the job training and quality assurance programs/initiatives 13.
