Including but not limited to: Examines patient using appropriate testing methods and instrumentation, appropriately records and interprets all findings, and identifies patient needs. Formulates a detailed assessment based upon findings of the examination. Develops treatment plan and patient outcomes, incorporating the input of the patient, their family and other health professionals. Ensures that the plan and outcomes are functional, time-based, and measurable, and incorporates diagnosis, prognosis, physical/mental status, and anticipated lifestyles. Safely and effectively implements the treatment intervention. Applies sound clinical judgment and skill. Engages a high degree of patient motivation and cooperation in treatment, as evidenced by patient compliance with treatment intervention. Provides patient/family education during therapy and in preparation for discharge. Conducts ongoing assessments of the patient´s status and revises treatment intervention, goals & outcomes as appropriate. Responsibilities Under the supervision of the Coordinators or Lead Therapist and working within their scope of practice as part of a multidisciplinary team, the Staff Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs that improve or restore physical function, alleviate pain and prevent dysfunction in patients with impairments, functional limitations, disabilities or changes in physical function and health status from injury, disease or other causes. Qualifications Education: BS, MS or DPT in Physical Therapy from an accredited program. Licensure, Certification, Registration: Active MA license. Skills, Knowledge & Abilities: Knowledge of Physical Therapy theory and practice. Ability to observe, assess and record symptoms, reactions and progress. Skills in critical judgment and thinking. Ability to effectively communicate medical information, test results, diagnoses and/or proposed treatment in a manner easily understood by the patient. Clear and effective oral and written communication skills. Knowledge of Physical Therapy services, facilities and equipment. Proficient in the use of a pc and a variety of electronic systems and databases for scheduling, data collection, reporting, communication, and other relevant activities. Ability to assess individual patient needs. Skill in maintaining patient medical record. Knowledge of range of therapy methods and techniques. Ability to maintain quality, safety, and/or infection control standards. Ability to use independent judgment and to manage and impart confidential information. In the absence of any of these skills, demonstrates the ability to be thoroughly trained to meet organization standards. Experience: Requires up to one year of on-the-job experience in a relevant environment, necessary to acquire and effectively utilize knowledge of department policies and procedures, and to fulfill quality and quantity standards for the job. Shift DAY/EVE Physical Requirements The position requires periods of concentrated mental and visual attention. Nature of work requires lifting or moving medium weight (over 20 and up to 75 pounds). Flow of work and character of duties require above average manual coordination. Flow of work and character of duties require high range of motion. Performance of duties allows for variation in positioning and tasks. Assignments require frequent repetitive motion. Working conditions include minimal exposure to elements such as odor, noise, dust, chemicals, temperature extremes or airborne pathogens. Performance of duties generally does not involve exposure to hazardous materials. Incumbent must be continuously prepared to prevent a patient fall. Assignments include transporting, transfer and positioning of patients. Position may entail exposure to bodily fluids and needle sticks, standard precautions must be used. Performance of duties may require the ability to hear soft and loud noises. Performance of duties may require the ability to discern and identify colors. Performance of duties may require the ability to discern sharp from dull and hot from cold.
