DESCRIPTION:

QUALIFICATIONS:

Required:
1. Graduate of an accredited school of nursing. Preference will be given to applicants with educational preparation appropriate to the immediate or long-range needs of the agency.
2. Current license to practice as a Registered Nurse in the State of Michigan.
3. Minimum of one to two years of med/surg, critical care or demonstrated experience in a specialty area.
4. Current Michigan driver’s license and proof of valid automobile insurance.

Preferred:
1. Two years hospital experience.
2. Experience in a certified home health agency.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Job Function
1. Initiates and provides medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition, achievement of the rehabilitation potential, and reduction of pain and anxiety.
2. Makes the initial visit to assess the patient’s physical and emotional status, needs, and resources through observation, interview and analysis of records.
3. Prepares the nursing plan of care, in accordance with the physician’s plan of treatment, based on problems identified in the nursing assessment and provides patient teaching using the Care Plan/Standard of Care.
4. Interprets to the patient and family the implications of the diagnosis and nature of the treatment, consistent with the Physician’s instruction.
5. Provides emotional support and encouragement to help the patient and family accept and adjust to the effects of disease or disability.
6. Supervises and evaluates patient self care or care given by others.
7. Notifies physician and Clinical Services Supervisor of changes in patient condition.
8. Completes and submits records, reports and other documentation related to the work.
9. Seeks to improve quality of service by continuously developing individual and unit competencies within scope of practice. May serve as preceptor for staff education.
10. Performs other related responsibilities as necessary.

POSITION SPECIFIC EXPECTATIONS:

Functions as Primary Nurse in assigned geographical area:

- Responsible for delivery of care to patients within area.
-Prepares a daily schedule of planned visits and establishes priorities on a weekly basis to assure physician ordered frequencies are met.
-Provides teaching of special procedures as needed.
-Prepares Home Health Aide plan of care form for each patient and makes supervisory visit to HHA’s patients every two weeks.
-Uses Medical Social Worker, Dietitian and therapists as resources and advises attending physician of patient’s need for other services when indicated.
-Confers with other health care personnel in planning, implementing and evaluating service to coordinate patient care.
-Responsible for the timely preparation of closing summaries to physicians and renewal of physician’s plan of treatment every 60 days with direction from Clinical Services Supervisor.
-Other duties as requested.

Miscellaneous

-Prepares accurate and complete clinical recording of the events of each patient visit including observations, treatment and other significant information.
-Provides nursing service in other geographical areas, as needed, to assist in meeting patient care priorities and balancing work assignments.
-Participates in the rotation of weekend, PM on call rotations and holiday work assignments. Insures continuity of care by promptly reporting events of visits made to staff nurse with primary responsibility for each patient seen.
-Participates in staff meetings including the development and evaluation of agency policies, procedures and program elements.
-Participates in staff in-service education programs and other opportunities for learning and plans for continuing self-development.
-Participates in clinical record review process.
-Assists in the orientation of new staff members.
-Represents the agency as a staff professional and interprets agency programs to community.
-Participates in the planned program of providing field observation to nursing students.
-Participates in mandatory recertification of CPR training.

-Must have the ability to hear and see the client in order to monitor their condition. Must be able to discriminate objects by color.
-Must have the physical ability to lift, turn and position clients.
-Must have the ability to work in extremes of hot and cold temperatures or in wet surroundings.
-Must have the ability to work with or near chemicals or other biological or allergenic substances used in wound care.
-Must have the ability to work near fumes such as wound odors, home odors, cigarette/cigar/pipe smoke.
-Must be able to work near radiation sources such as patients post radiation, receiving RT, cobalt, etc.
-Must be able to wear protective clothing equipment such as gloves, masks, respirators, isolation gowns, etc.
-Physical and emotional health and maturity to function effectively.
-Ability to read, write, and carry out written / oral directions.
-Must be knowledgeable of good body mechanics and practice safe body mechanics.
-Ability to stand, walk, crouch, stoop, bend, climb, kneel and drive for periods up to two-hour increments.
-Ability to lift 5-25 pounds above head.
