POSITION REQUIREMENTS

ESSENTIAL FUNCTIONS:
1. Assesses patient nutritional status appropriately, considering all pertinent factors.
2. Evaluates patient nutritional needs appropriately.
3. When able, interviews patient/patient's family appropriately to obtain diet history and other pertinent information relating to nutritional needs.
4. Develops an appropriate care plan for patients determined to be at Nutrition Risk.
5. Performs follow-up evaluation of patient nutrition status, as determined by policy.
6. When required, provides nutrition/diet education to patient and/or significant other, including information relating to patient's readiness to learn, response to education (how demonstrates knowledge), ability to purchase and prepare foods and, if needed, ability to incorporate ethnic foods. Use appropriate education materials.
7. Follow-up education is provided when appropriate.
8. Enters all pertinent information in the Progress Notes of the patient Medical Record using the appropriate format.
9. Uses appropriate hospital abbreviations.
10. Documents patient care and patient education given on Care Plan Form.
11. Reviews list of patients receiving medications with potential for significant food-medication interactions and adjusts meal plan as needed.
12. Demonstrates knowledge of enteral and parenteral nutrition as evidenced through peer review and annual test.
13. Interacts effectively with Physicians, Nursing, Pharmacist, Speech Therapist and other staff as needed in an interdisciplinary setting to provide continuity of care to patients.
14. Participates in Hospital Outreach Groups when nutrition information is requested.
15. Serves as a resource person for Nursing, Physicians, Dietetic Assistants and other hospital staff as needed.
16. Appropriately offers substitute menu items (from approved write in list) to improve patient satisfaction.
17. Communicates verbal and written information effectively and clearly to staff.
18. Assists with data collection for CQI and QM projects and participates in meetings as needed.
19. Participates in Discharge Planning Conferences and other committees as requested.
20. Participates in and contributes to clinical nutrition projects such as Nutrition Displays and Nutrition Month Activities.
21. Assists in the Dietetic Technician position when deemed necessary (i.e. staff shortages) to ensure appropriate meal service to patients.
22. Prioritizes time appropriately, completing activities in a timely manner, determining order of patient service and documenting patient care information.
23. Participates in continuing education to maintain clinical nutrition skills. Production staff, educates and teaches peers in regards to cooking techniques, attends staff meetings, leads by demonstration.
24. Assists in special projects as assigned by the production manager and director. Demonstrates skill when preparing items for catering.
25. Contacts with the community are conducted in a manner that supports the system philosophy and reflects positively on the system programs.
26. Participates in development activities to support the individual unit / department.
27. Maintains and promotes a safe environment by adhering to general and departmental safety policies. Promptly reports all potential safety risks and completes appropriate safety paperwork when required.
28. Able to locate and refer to CRH/MGMC and unit-specific policies and procedures, including fire, safety, and infection control manuals.
29. Supports the system Infection Control policies.
30. Provides education for the department and / or peers within area of expertise.
31. Participates in monthly journal clubs and departmental meetings.
32. Takes call, as needed, according to schedule.

EDUCATION, EXPERIENCE AND SKILLS

1. Baccalaureate Degree in Dietetics from accredited college
2. Experience in Hospital Clinical Nutrition preferred
3. Computer knowledge helpful

LICENSURE/CERTIFICATION

1. Registered with / Commission on Registration with ADA
2. Food Handlers Card-within first 30 days of employment
