Job Summary:
Within the context of a client and family-centred care model and in accordance with College of Registered Nurses of British Columbia (CRNBC) standards of practice, Code of Ethics, and the established mission, vision, values, objectives and policies of the organization, works in close collaboration with Community Intake, Home Health Scheduling and Team Leaders, the Home Health Clinical Coordinator is responsible for coordinating the workload of the Home Health Interdisciplinary Team. Provides consultation and advice to interdisciplinary staff on client care issues to ensure effective and efficient delivery of Home Health Services in clients' homes or in the Nurse Managed Clinics. In consultation with members of the Home Health leadership team, organizes priorities in response to fluctuating workload volumes and complex clinical situations. Works collaboratively with team members to address health care needs. Provides intake and scheduling support for coordination of flow of care through consultation and determining care needs, developing plans for client flow, discussing services provided and providing follow up with clinical leaders and Home Health Manager.

Qualifications:
Baccalaureate in Nursing and three (3) years recent, related experience as a home care nurse or an equivalent combination of education, training and experience; current practicing registration with CRNBC. Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.
(Comprehensive knowledge of provincial acts and regulations related to Home Health, Home Care Nursing, Case Management, HSCL, and CSIL Programs. Knowledge of community resources. Broad knowledge of the principles, practices, and procedures of Case Management to medical, surgical and palliative care nursing, OT, PT, Care Coordination and Home Support. Knowledge of family dynamics and coping mechanisms.)
