Description: • Completion of an approved NP program
POSITION SCOPE:

The PHCNP works collaboratively with an off-site family physician(s) and on-site paramedics to provide a range of primary care services which encompass health promotion, injury and illness prevention, diagnosis, treatment and monitoring of commonly encountered health problems and injuries, monitoring of stable chronic health problems; and community development and advocacy. These services are provided within a holistic, client-centered care framework.

The primary health care (PHC) NP assumes responsibility and accountability for the nursing assessment and/or primary care for a selected population of patients. S/he is responsible to maintain currency of licensure and certification and function under the Collaborative Practice Agreement (CPA) with a collaborating physician(s) by consulting with the collaborating physician and other health professionals in accordance with the CPA. The NP will effectively communicate with all members of the health care team, the public and external community partners.

QUALIFICATIONS:

• Registered or eligible to register with active-practicing status, as a Nurse Practitioner with the CRNNS
• One year NP experience in primary health care or a reasonable combination of nursing experience and education
• Level of competence to practice within the Nova Scotia NP Scope of Practice
• High level of self-motivation and initiative
• Demonstrated leadership skills and abilities in a clinical setting
• Demonstrated health promotion skills
• Demonstrated above average written and verbal communication skills
