Mercy Gilbert Medical Center is a four story, full-service, acute-care hospital spanning 60 acres. An additional 40 acres housing a medical office building, research and lab facilities will complete the campus. With 41 hospitals in Arizona, California and Nevada, CHW is one of the largest healthcare systems in the West. POSITION REQUIREMENTS ESSENTIAL FUNCTIONS: 1. Overall patient satisfaction as measured by hospital tool. 2. Maintains professional licensure in a current status as defined by the minimum qualifications of this position. 3. Manages routine assignments independently, consistently prioritizing when managing assignments. 4. Consistently introduces self and role when interacting with patients and families. Anticipates patient needs as evidenced by frequent updates to patient and family, notification of unexpected or expected delays. 5. Consistently utilizes scripts both in face-to-face patient/family interactions and telephone interactions. 6. Patient's pain is assessed and documented. 7. Documents complete care of patient. 8. Directs and supervises assistive personnel (including, but not limited to, PTA's and PT Techs). 9. Collaborates in data collection and implementation of ongoing performance improvement monitors. 10. Performs an appropriate assessment on each patient as related to the therapy requested and completes reassessments as per policy. This includes pediatric, adolescent, adult and geriatric patients. 11. Evaluates, implements and modifies the plan of care based on observable patient responses and attainment of expected outcomes. 12. Performs preparatory data gathering to adequately evaluate the patient's need prior to initial patient contact; reviews patient records; confers with treating physician; collaborates with nursing staff and other members of the health care team regarding each patient. 13. Demonstrates knowledge of, and complies with practices and plans: Emergency/Disaster Plan/Standard Infection Control Practices/Safety Policies. 14. Maintains confidentiality of records, employee records, hospital information system, and administrative information. Respects co-workers privacy by refusing to gossip. Conversation between employee and customers are conducted with discretion. Adherence to privacy/HIPAA policies, maintains confidentiality of charts/medical records, avoids "open" discussion of patients/patient status. 15. Demonstrates knowledge and proficiency of physical therapy modalities. 16. Participates in patient and hospital safety and quality programs: fall prevention program/patient safety initiatives. 17. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. 18. Ensures that patient charges are accurate and submitted in a timely manner. 19. Demonstrates adherence to the Standards of Conduct. 20. Completes and turns in Standards of Conduct acknowledgment card. 21. Fulfills compliance education requirements as directed by the Compliance Department or department management. 22. Shows interest in job, is cooperative, pleasant and efficient, responds professionally to constructive criticism, offers assistance, is friendly and takes time to listen. 23. Promptly shows courtesy to all customers in greeting and conversations. Seldom holds personal conversations in front of customers. Only urgent personal phone calls interrupt work. 24. Expresses pride in being a member of our healthcare team. Conduct is a positive reflection on our hospital. Takes responsibility for safety of work and surrounding area.
