Hours: Generally a 40-hour per week position, Monday - Friday but subject to change based on needs of the department. Will include On Call hours Requirements: Masters Degree and Nurse Practitioner Certification through ANCC required. Current State of CT APRN licensure required. State of CT -Controlled Substance Registration for Practitioner is required (Issued by the Dept of Consumer Protection). Minimum of 2 years of cardiac surgery, acute care/critical care, transplant or VAD experience preferred. Will also need to obtain Adjunct Medical Staff Privileges at Hartford Hospital if hired. Job Description: Coordinates and participates in the Clinical management of potential and post VAD patient s in collaboration with physicians. The position oversees coordinating these patients across the continuum, and requires the ability to interface successfully with patients & their family; physicians (heart failure physicians, cardiologists, cardiothoracic surgeons, and referring physicians outside the institution); heart transplant coordinator, nursing and other clinical staff. This role includes but not limited to coordinating clinical evaluation of potential VAD candidates, coordinate discharge and manage the patient post - VAD placement in outpatient clinic. Other activities include educates nursing staff and patient/caregiver on patient care; maintaining VAD equipment safely; and research. This role will also provide education and support to the community who will support these patient post discharge (EMT, home health nurses and other health care professionals, ETC.).
