Description:
This position is located in the Infection
Public Health Nursing Department and is under the direct
supervision of the PHN Director or designee.
As a member of the healthcare team, performs a variety of patient care activities to assist in providing comprehensive health care services in accordance with established program policies, standards, procedures and compliance with local, Tribal, state and federal regulations MINIMUM QUALIFICATIONS: 1. Completion of an accredited program of study leading to a diploma in Practical Nursing 2. Current and valid State of Arizona license as a Practical Nurse 3. Must have at least 2 years previous nursing experience; Public Health or Community Health preferred 4. Current CPR , and BLS certification, or ability to obtain certification within 30 days of hire 5. Current and valid State of Arizona driver's license ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Provides home visits to reinforce teaching goals related to the disease process, medications and identification of health related needs 2. Evaluates patient's response to medication and treatments including compliance with care plans 3. Observes, collects and records a variety of routine patient data; reports observations to Public Health Nurse and/or provider as appropriate 4. Provides nursing care within scope of practice; assists with development and application of nursing care plan 5. Administers specific medication, vaccines and other types of injections in accordance with standing orders 6. Fills medisets and syringes as requested 7. Identifies and reports unusual findings or observations, clinical symptoms, behaviors and treatment reactions to Public Health Nurse and/or provider as appropriate 8. Assists patient to and from facilities and other clinic sites for procedures 9. Stocks/restocks nursing service areas as needed 10. Performs visual inventory of stock items; re-orders standard items to provide for availability of supplies for optimum efficient unit operations 11. Participates in a variety of in-services and unit meetings to maintain understanding of care plan changes 12. Refers problems beyond standards of practice to Public Health Nurse as appropriate 13. Assists in the implementation of annual health fairs, vision/hearing checks, immunization clinics and wellness screening programs 14. Assists schedule appointments to provide for efficient operations 15. Documents visits and assists maintain records and reports of activities 16. Participates in departmental orientation, on the job training and quality assurance programs/initiatives 17.
