GENERAL STATEMENT OF DUTIES:

Responsible for evaluations on all referrals for admission according to established criteria; develops referral sources through ongoing contacts with agencies in specific areas; represents this facility to multiple health agencies and referral sources in order to market services and programs.

SUPERVISION RECEIVED:
Directly responsible to the Director of Resource Management.

SUPERVISION EXERCISED:
May provide technical supervision to the Central Registration Staff (1.5 FTEs)

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Independently evaluates referred patients for possible admission to this facility using specific developed criteria.
Responsible for the completion of a comprehensive written and on-site assessment incorporating the overall current and past medical and rehabilitation data and anticipated goals of the patient, family and physician.
Responsible for independent on-site decisions concerning admission when appropriate. Consults with the Director Resource Management or other appropriate disciplines to facilitate the admission decision process, on occasional difficult referrals.
For those patients who meet the admission criteria, provides immediate notification to the referring facility and the admissions department; obtains pre-certifications and presents written and verbal treatment plan per insurance policy. On occasion may need to contact the nurse manager directly.
Notifies appropriate departments of any anticipated special needs or equipment for those patients who do not meet the admission criteria, effectively communicates to the acute facility and/or family/ physician, the reason for rejection.
Obtains a confirmation of discharge to this facility and facilitates a timely admission process.

PRINCIPAL DUTIES AND RESPONSIBILITIES (continued)
Establishes a realistic pre-admission discharge plan and anticipated length of stay.
Develops and maintains an on-going professional rapport with assigned territory of facilities.
Serves as a functional representative of the hospitals programs and services where considerable discretion and resourcefulness are required.
Determines level of care and program assignment accordingly.
Compiles weekly statistics for Director Resource Management.
Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, and co-workers.

KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED:
RN, or other healthcare professional degree, required; BSN, CRRN preferred.
Knowledge of community services to establish anticipated discharge plan goals.
Three (3) to five (5) years of healthcare experience.
Rehabilitation experience preferred.
Current registration in Massachusetts.
Current Massachusetts drivers license and demonstrated good driving record.
Demonstrated interpersonal and marketing skills.
Demonstrated ability to make independent decisions.
Demonstrated organizational skills.
Excellent written and communication skills.
Knowledge of insurance regulations.

Understands and applies the criteria under Managed Care.
Understands and applies the criteria for assignment of specific programs.
