Great opportunity for an experienced Nurse
Practitioner!
CHD Meridian Healthcare, a subsidiary of

currently seeking a Full-time Nurse Practitioner
for our wellness center located in Gary, IN.
This position works collaboratively with our healthcare
team to provide quality care. The successful candidate
will have an excellent knowledge base to utilize
while performing examinations, assessments, treatments,
and appropriate referrals.
This position requires a FNP or Adult NP. MSN, current
IN license, and certification by the AANP, CRNP,
or ANA required. Minimum 2 years Occupational, ER,
Wellness, or Primary Care experience is preferred.
Phlebotomy and good computer skills are required.
Serving over 150 clients at over 350 locations nationwide,
