Geriatrician needed for Post Acute Care Discharge
Clinic
This position coordinates and administers
the clinical management function for the assigned health
plan patients discharged from a skilled nursing facility,
to a clinic/home setting.
The physician provides
clinical care expertise while working in conjunction
with the medical and case management staff and the
Medical Director. These responsibilities will include
medical management and quality care for the assigned
patients.
