Established in 1999, Geisinger's Palliative Medicine Department provides inpatient and outpatient palliative care with an overall goal of assuring the patient's dignity, autonomy, and comfort. This nurse practitioner will work in a collaborative practice with the department physician staff in assessing needs, planning and providing education and care, evaluating outcomes and coordinating follow-up and referrals for patients. Meets the qualifications to provide care for patients in the specific age range in the Clinic. Reports clinically to the Department Director and administratively to the Operations Manager. Essential functions include, but are not limited to, the following: • Serves as a clinical expert, providing direct client care including assessment, diagnosis, treatment and plan of care. • Participation in rounds and the call schedule. Some travel is required. • Designs and provides both formal and informal education programs to the medical staff, students and community in addition to the education provided to patients, families and caregivers. • Accurate and complete charting including proficient electronic medical record documentation. • Serves as a leader and role model for the professional development of peers, colleagues and other health care providers. • Self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency within specialty range. This Nurse Practitioner collaborates with the physician for all patient related responsibilities and deals directly with medical staff members, nursing personnel and other departments to coordinate patient care requirements. Fosters relationships with ancillary and consulting staff. Demonstrates a strong desire to work with end-of -life patients displaying the knowledge and the intervention of a caring health professional to bring resolution and comfort to their patients and loved ones. Requires a certificate of completion from an approved program for Nurse Practitioners and must be eligible for or certified by the Pennsylvania State Board of Nursing. Minimum of 2 years clinical nursing experience required. Previous and end-of-life care preferred.
