We are currently
seeking a Clinical Pharmacist:
Responsible
for the evaluation, interpretation and management of
patients in specialty clinics (i.e. Anticoagulation
Clinics, Lipid Clinic). Work collaboratively with physicians
and other health care providers in diagnosing and treating
patients. Provide other functions to ensure that the
use of pharmaceutical agents are rational and cost-effective.
Excellent organizational, interpersonal and decision-making
skills. Knowledge of all Microsoft software. Capable
of working
independently.
Required: Current
California Pharm.D. license. Master's degree from a
four-year college and/or a professional certification
requiring formal education beyond a four-year college.
Pharmacy residency or fellowship preferred. Board certification
in Pharmacotheraphy specialty preferred. Three years
of clinical pharmacy experience in hospital or ambulatory
care; or completion of a 1 or 2 year accredited pharmacy
residency or fellowship program. Knowledge of all Microsoft
software. Excellent organizational, interpersonal and
decision-making skills. Ability to work independently
Preferred:
5 years of experience in a hospital or ambulatory care
as a Clinical Pharmacist.
