Job Details: Clinical Social Worker

Description: This position is located at the Gila Crossing Clinic and is under the direct supervision of the Clinic Director or designee.
ESSENTIAL FUNCTIONS:
1. Provides individual, family and group therapy including, but not limited to:
psychosocial assessment, crisis intervention, diagnostic, therapeutic and preventative care in accordance with established plan of care.
2. Reviews charts of assigned patients to determine appropriate referral services
based on recommended treatment plan.
3. Provides individual, family and group counseling and education to patients
within context of cultural beliefs and norms.
4. Participates as a member of a multi-disciplinary team meetings to foster
collaboration and communication.
5. Maintains progress notes in patient chart of all contacts within established
timeframe.

MINIMUM QUALIFICATIONS:
Master's degree in Social Work or closely related field; equivalent combination of education and experience may substitute. Current State of Arizona license in behavioral health field; CPR certification within 30 days of hire. Must have at least 2 years previous counseling experience; Native American population preferred. Current valid State of Arizona driver's license; fingerprint clearance to work with children upon hire.
