The incumbent is responsible for providing cardiovascular and lifestyle risk factor management to military health care beneficiaries seeking services in the Integrative Cardiac Health Project's (ICHP) Cardiovascular Prevention Program (CPP) at Walter Reed Army Medical Center utilizing a collaborative team approach. Work in collaboration with the clinical team to assess, screen and evaluate the patient's cardiovascular profile. As a consultant in the patient's plan of care, works to maintain open communication with the patient's additional providers and to facilitate quality care within the cascade of success. As the patient's preventive health coach, works to identify motivators and triggers in the patient's lifestyle change plan and makes appropriate recommendations to maximize patient success. Responsible for following CPP clinical care guidelines to track and maximize identified patient outcomes on an individual as well as aggregate level. Understands that behavior change demands a synergistic approach of multiple disciplines and educates the patient appropriately. Provides complete customer service to patients while providing comprehensive cardiovascular disease prevention. Effectively collaborates with the clinical team as well as the Executive Team to optimize the efficacy of the CPP and ultimately ICHP as a whole. Registered nurse practitioner licensed from the appropriate state board of nursing; experience in cardiology or cardiovascular nursing is preferred. Must have adult education skills, excellent organizational and communication skills and the ability to work in a participative team-oriented environment. Current certification in BLS required; certification in Basic Cardiac Life Support preferred. Registered Nurse Licensure in the District of Columbia, a state or territory of the United States and Certification as Adult or Family Nurse Practitioner. Minimum Education/Training Requirements: Masters in Nursing which includes formal training as an Adult Nurse Practitioner or Family Nurse Practitioner. Minimum of 1 year's clinical experience as a nurse practitioner and certification from the appropriate national licensing agency. Minimum Experience: At least one years experience as an Adult or Family Nurse Practitioner; experience in cardiology or related specialty preferred.
