Description:
The pharmacy department at Guelph General
Hospital is progressive and functions in a dynamic,
team-oriented hospital setting.
. We demonstrate
a commitment to patient care, patient safety and continuous
learning. The pharmacy technicians perform most of
the medication distribution, including order entry,
dispensing and checking to enable the pharmacists to
practise on the patient care units. The hospital is
currently using on-line documentation, automated unit
dose packaging and automated medication distribution
(Omnicell).
We currently have a full-time employment
opportunity for a Pharmacy Clinical Coordinator. This
position is responsible to use evidence-based practices
and the principles of pharmaceutical care to ensure
a safe and effective medication therapeutic plan for
Critical Care patients. In addition to providing direct
patient care, the Pharmacy Clinical Coordinator oversees
and mentors pharmacy students and the staff pharmacists'
patient care activities and is an active member of
a variety of hospital committees.
Other duties
of the Pharmacy Clinical Coordinator include implementing
and evaluating patient care paths, clinical practice
guidelines and other initiatives, participating in
the review and the development of relevant institution
and departmental polices, and procedures, and responding
to requests for drug information and consultations
using critical appraisal skills and clinical judgement.
The successful candidate will be self-motivated,
with strong communication and problem-solving skills
and will be licensed (or eligible for licensure) in
Part A, the Ontario College of Pharmacists. Hospital
experience, an advanced practice pharmacy degree, e.g.
Pharm D and a hospital residency, or equivalent, are
an asset.
