Job Details: Pharmacist

 
Description: Looking for a contract Pharmacist with a minimum of 4 years of retail or outpatient pharmacy experience.
- Check prescriptions for patient eligibility, drug availability, usage, dosage, mode and rate of administration, completeness of directions and possible incompatibilities noted from referencing the patient's medication profile.
- Fill prescriptions, including controlled drug prescriptions, by selecting the correct drug and dosage form, reducing the drug from bulk to the quantity prescribed, compounding when necessary, and packaging the drug in appropriate container.
- Exercise care in the dispensing of drugs, especially controlled substances, to assure that legal and agency regulations and instructions are followed, that proper and complete record keeping is maintained, and that the drug storage areas are stocked, safe and secure.
- Notify the prescriber, who may be on staff or a community physician, when there are questionable dosages or possible therapeutic incompatibilities based on a review of the patient's medication profile.
- Consult with and advise members of the medical staff on matters such as contraindications, side effects, and adverse drug reactions; and suggest alternative medications as appropriate.
- Furnish guidance to prescribing physician on current MEDDAC Pharmacy and Therapeutics Committee policies, applicable federal and state laws, and stocked formulary and non-formulary item information.
- Dispense prescriptions to patient, counseling the patients concerning their medication, i.e., uses, side effects, directions, etc. in accordance with the Omnibus Budget Reconciliation Act (OBRA) guidelines.
- Perform other duties as assigned within the general scope of those normally performed by a registered pharmacist. Requisitions, receives, and maintains pharmacy supplies at appropriate levels according to pharmacy SOP's. Inventories controlled substances in accordance with pharmacy SOP's.
- The contract pharmacist shall provide technical guidance to and act as preceptor for the pharmacy technicians; assume technical responsibility for all work performed by technician(s) under the technical guidance of the contract pharmacist; check the work of pharmacy technicians to assure that all drugs dispensed are properly labeled and packaged, and that directions for use and cautions, when necessary, are complete before the drug is dispensed to the patient.

- The contract pharmacist shall be a graduate of an accredited college of pharmacy.
- The contract pharmacist shall have and maintain a valid unrestricted state license in any state in the United States, District of Columbia or the territory of Puerto Rico.
- The pharmacist shall not have a history of license revocation, suspension or encumbrance due to misuse or improper dispensing of controlled substances.

- The contract pharmacist shall be able to type a minimum of 20 words per minute.
- The contract pharmacist shall have a minimum of 4 years of retail or outpatient pharmacy experience.
- The contract pharmacist shall have current Basic Life Support (BLS) certification from the American Heart Association.
