Evercare has been awarded a contract for the State of Hawaii's Quest Expanded Access Program for the aged, blind and disabled, and will be proud to serve an estimated 16,000 individuals on the islands of Kauai, Oahu, Maui and Hawaii. The hired Care Manager will determine the medical needs of high-acuity Medicare and Medicaid members.

Primary responsibilities include:
Working with low-risk patients who have chronic conditions
Performing health-risk assessments
Setting up DMEs and making sure authorizations are approved
Documenting all cases in our system
Coordinating member care

Qualifications
Candidates must have a current, unrestricted LVN license in the state of Hawaii or have a degree in social work. General computer skills are necessary. The ability to have a strong phone presence and excellent communication skills is critical. A solid medical background is essential. Those who are bilingual in Chinese, Vietnamese, Samoan, Filipino, or Japanese are especially encouraged to apply, as are those familiar with Medicare or Medicaid.
