Be

to work with the acute care departments to achieve
performance goals matching Park Nicollet values. Case
managers will be responsible for reviewing and monitoring
the care and plan of care all patients assigned to
on the unit(s) they work. Case Managers will recommend
and initiate strategies to progress those patients
towards discharge as soon as possible.
BSN preferred
with current RN license. Minimum of four years full-time,
hospital-based acute care nursing experience. Two years
full-time experience in any of the following areas:
utilization management, case management, home care
setup, discharge planning.
