Description:
Graduation from an accredited school of pharmacy
with PharmD (or B.S. with equivalent clinical experience).
Licensed or eligible for licensure in the State of
Ohio as a Registered Pharmacist.
Minimum of 1 year of clinical pharmacy experience, preferably to include managed care or ambulatory care experience. Completion of residency program preferable in ambulatory care. Exceptional communication (verbal and writing skills) as well as ability to establish and maintain working relationships with other health care providers with minimal supervision. Strong PC skills, including Microsoft Office software essential. The position scope of the Clinical Pharmacy Specialist Primary Care includes but is not limited to the following abilities: Design, coordinate, and implement clinical programs to improve patient care, including but not limited to:: pharmaceutical care clinics, therapeutic drug monitoring programs, therapeutic guidelines, and programs to support national and regional clinical initiatives. Provide pharmaceutical care for patients with direct supervision of referring physician with completion of appropriate documentation in electronic medical record. Apply appropriate techniques and strategies to effectively respond to drug information requests Assist in staffing the Anticoagulation Clinic and/or the Pharmacy Utilization Management Service according to approved policies and procedures Develop formulary recommendations in conjunction with medical staff. Present evaluations and recommendations to the P&T Committee and communicate and implement formulary decisions. Critically evaluate medical and pharmacy literature Posting
