Graduation from an accredited school of Pharmacy
with a Bachelor of Science or Doctor of Pharmacy (Pharm.D.)
degree.
Residency or fellowship training in formulary management, drug information or a related field or equivalent clinical experience preferred. Licensed or eligible for licensure in the State of Ohio; maintains ongoing continuing education requirements. Two years licensed pharmacist experience. Proficient with computers and their applications, including experience with or ability to learn Microsoft) Office software including Word, Excel, and Access. Exceptional communication (verbal and writing) skills. The position scope of the Clinical Pharmacy Specialist, Formulary Management includes but is not limited to the following abilities: Appropriately review and respond to Requests for Drug Coverage according to approved policies and procedures and medical exception drug use criteria. Complete all required documentation and communications within required time frames. Recommend potential formulary alternatives if request does not meet criteria. Formulary Management: Support drug formulary management including development of evidence based criteria to evaluate requests for drug coverage in conjunction with physician chiefs and the P&T Committee. Develop formulary recommendations and guidelines in conjunction with medical staff. Present evaluations, recommendations and guidelines to the P&T Committee and Regional Guidelines Committee and communicate and implement formulary decisions within the scope of the PUM Service Utilization Project Management: Assist in the coordination and implementation of drug use initiatives to support regional goals and promote quality, cost effective drug therapy. Track and report utilization of targeted medications and medication classes and evaluate impact of prescribing practices of individual physicians and departments to assure rational, cost effective prescribing. Drug Information Service Support: Respond to drug information questions from healthcare professionals. Ability to critically evaluate medical literature. School of Pharmacy Student Support: Participate in school of pharmacy activities including preceptorship of students participating in clinical rotations in managed care and drug formulary management services.
