We are currently seeking a caring and experienced RN to become our newest Hospice On-Call, RN. Primary responsibilities include:
Conducting visits to hospice patients' homes or skilled nursing facilities
Coordinating, assessing and implementing care plans for these patients
Documenting patient services in an accurate and timely fashion
Attending weekly interdisciplinary meetings
Providing emotional support to patients' family members

Qualifications
You must have a current and valid license as an RN in Arizona to be considered for this position; clinical experience is also essential. Home and/or hospice care experience is preferred, and a background in long-term care is a plus. Basic computer proficiency is also necessary.
