Summit ElderCare
(SE) - a Program of All-inclusive Care for the Elderly
(PACE) assists elders to remain living in the community
while providing necessary support for both them and
their care givers. Sponsored by Fallon Community Health
Plan (FCHP), a Health Services organization with a
mission of making our communities healthy, it is the
only program of its kind in central Massachusetts.
Nurse Practitioner - 30 Hours per week (candidates
seeking FT position will also be considered)
The
successful candidate will provide primary health care
to PACE program participants focusing on disease prevention,
wellness and management of chronic and episodic illness
in collaboration with the program's Primary Care Providers
and Geriatric oriented Interdisciplinary Team (IDT).
Contribute to the IDT problem solving and decision
making processes to develop and manage individual plans

. Positions include some on-call responsibility on
a rotation basis.
Requirements
Must be
a licensed RN, graduate of an accredited Nurse Practitioner
Program, and certified as a Geriatric, Adult or Family
Nurse Practitioner. One to three years of Nurse Practitioner
experience preferred with at least one year of experience
in geriatric care or care of disabled.
