Colo. Medical License, DEA Cert., Board Certified in Fam. Medicine or Obstetrics/Gynecology. Obtain Medical Staff privileges within the Centura Hosp. System (SAC & SAN). 3 yrs prior exp. in a Family Practice or OB/GYN setting.
