Job Details: Clinical Services Nursing Supervisor

Description: The professionals at Visiting Nurse Services of MI, a subsidiary of Mclaren Health Care, understands the philosophy of delivering exceptional care and have been doing this for more than 100 years.
Daily operations needs may include:

Supervision and evaluation of clinical team for clinical appropriate and fiscally responsible care. Provides on-the-job training in deficient areas.
Selects, trains/orientates, assigns staff, evaluates performance, and makes recommendations for personnel actions to include discipline and discharge.

Participates in the grievance resolution process to include investigating and responding to first step grievances.

Plans and coordinates assigned patient caseload.

Case Management to include coordination of patient visits, evaluation of patient needs, directs skilled services according to care plan/standard of care and regulatory requirements (i.e. COPs, BCBS Interqual criteria), and integration of pathways/telehealth into patient care plans. Provides written plan of care and revises as necessary. Notifies physician and team members of changes in patient condition.

Promotes interdisciplinary approach to patient care, acts as a resource to assist the clinical team in the planning, implementing and coordinating of services including leading weekly team conference for assigned team.

Evaluates the provision of patient care and accuracy of supporting documentation through routine and random patient clinical record reviews/audits including but not limited to; OASIS, ICD-9 coding, Plans of Care/485, and initial physician orders.

Assist in performance improvement activities and PI plans (working with PI/ED department) including but not limited to OBQI, PI current goals (including homecare compare and Casper reports), Patient Satisfaction initiatives, Adverse Events, quarterly chart reviews, OASIS education plans, and infection control.

Oversees the quality and appropriateness of services and staffing as well as their availability to meet patient care needs. Monitors and addresses appropriately continuity and productivity.

Assists with organizational adherence to professional, regulatory (i.e. CMS, MDCH, 3rd party payers, JCAHO, OSHA) and accrediting body policies, procedures, requirements, and standards. Interprets and translates agency policies and procedures to staff.

Assists with problem solving of service operations and creates systems to monitor compliance within department.

Assists, prepares and presents at staff meetings.

Communicates effectively with staff including:
Day to Day communication to effectively work with staff Relates certified strategic plan to staff in appropriate day to day issues
Leading/assisting with employee focus committees
Leading disease specific teams for improvement of clinical practice and engage interest of employees
Assists and participates in promoting health programs to the community and represents agency in selected activities.
Assumes beeper responsibilities on a rotational basis.
Performs other related responsibilities as needed.

Requirements of the position include:
-Current license to practice as a Registered Nurse in the State of Michigan.
-Two years of acute care experience.
-Two years of progressive supervisory experience.
-One year experience in a Home Health and/or Hospice setting.
-Written and verbal communication skills and demonstrated ability to lead committees or teams.
-Valid Michigan driver's license and proof of current automobile insurance.
