Position Summary: This is a new role in an expanding heart failure and transplant program. Responsibilities will include overseeing the pre- transplant process and care of these patients as well as the post transplant patients. This will involve the care of patients in both the inpatient and outpatient setting. This position will include clinical management of patients across a continuum starting with advanced heart failure care to post transplant care. This position will be a part of a multi-disciplinary team and well as be able to coordinate care with internal and external team members to provide comprehensive patient care. Dedication, strong clinical skills, autonomy and excellent communication skills are a must.

Position Requirements: Must have a Master's degree and APRN license in the state of Connecticut. Preferably with 6 mos to 1 year experience managing a cardiac/critical or transplant population. Must have prescriptive privileges in the State of Connecticut. Advanced computer skills or willingness to advance skill or learn new programs is required. Will also need to obtain Adjunct Medical Staff Privileges at Hartford Hospital.
