You'll enjoy working with and getting to know a diverse mix of delta residents and experience an array of medical conditions and procedures you would normally not encounter in the lower 48 states. Come join us at Yukon-Kuskokwim and grow professionally, mentally and physically. Live our mission and our passion for care. Our staff is our most valued asset. Providing our patients the care they need a home away from home. Making them feel safe, warm, secure. You're the heart, the core of our business. YKHC is looking for an individual to help to provide first class care to our patients. If you are interested in helping to insure that YKHC is the best in providing quality care, in a caring, conducive environment then this is the position for you! Additional degrees such as a MPH or health related degree are a bonus. 2. candidate must have a minimum of 2 years experience in a cross cultural setting 3. candidate must have a minimum of 1 year experience in diabetes prevention/case management and supervisory experience We are really looking for someone who has experience working with grants and who is comfortable working in a dynamic environment--we are evolving how we deliver diabetes disease management to involve the Health Aides and we need someone who can work with my CHAP department and design a working model. We are struggling right now because the staff is really focused solely on the grant objectives and is resistant to this change. We need someone who will be able to engage the staff and get their buy in for this new model. We still need to function within the constraints of the grant, but this model is needed for sustainability. Qualifications: Degree in nursing, pharmacy, or nutrition. Preferred Bachelor's or Master's degree in Public Health, Education, Nutrition or related health field. Two (2) years experience in a cross-cultural diabetes education required. Preferred experience in the following: supervisory and staff management, case management, RPMS/database management, grant and budget management, working in a cross-cultural, rural Alaskan setting. RN, RPH, or RD required. Valid Certified Diabetes Educator (CDE) certification required or ability to obtain within two (2) years of hire. Preferred Basic Life Support (BLS) certification. Program planning; grant writing and management, public relations, effective communication skills (oral and written), organizational skills, and ability to chair meetings and teleconferences. Ability to effectively utilize Microsoft Office suite software, Internet, email, and RPMS/statistical/data analysis software. Preferred bilingual (Yup'ik/English).
