Description:
The Hospitalist Program Manager leads the
collaboration process which assesses, plans, implements,
coordinates, monitors, and evaluates options and services
to meet an individual's health needs through communications
and available resources.
The Hospitalist Program Manager supports the mission, vision and values of Medical Center of Arlington and the philosophy of the Department of Nursing and supports the nursing staff's primary commitment to protect the health,s afety and rights of the individual, family or community. The responsibilities are, but not limited to, the following: - Collaborate with Hospitalist Physicians to coordinate care of patients, emphasizing patient safety as the primary goal. - Practice autonomously consistent with practice standards and research based policy/procedures. - Exercise independent judgement within the context of interdisciplinary care. - Prioritize goals and development a plan to achieve clinical and sical outcomes. - Monitor various aspects of client's plan of treatment. - Act as a facilitator between physicians, payors and patient/family to coordinate the client's plan. - Interact with the health care team to implement identified goals. - Follow clients throughout the continuum of care for outcomes management. - Communicate effectively with physicians and care givers. - Provide strong patient advocate skills - Promote effective resources utilization and appropriate levels of care. - Interface effectively with case management staff and processes. - Develop community resource relationships. - Develop effective relationships with physician offices and staff. - Anticipate patient discharge needs. - Effectively manages time to meet the needs of the program. - Province hospitalist program leaders program outcomes data. - Action demonstrate protection of patient privacy and compliance with HIPPA regulations. - Collaborate with third party payors internal case management/disease management departments as appropriate. - Act as liaison for the program both internal and external to the facility. - Facilitate case conferences when appropriate to patient/family needs. - Provide patient education and discharge teaching as appropriate. - Participate in health promotion/educational community involvement, life time learning and nursing research. - Participate in development and evaluation of professional practice by participating on unit and/or hospital committees, special projects and attendance at staff meetings and town halls. - Actively participate in unit and/or hospital performance improvement activities - Attendance and being on time, ready to work your scheduled hours is an essential job function.
