Description:
Full-time opportunity to join our dynamic
team of Home Health Physical Therapists at PeaceHealth
Sacred Heart Medical Center.
Duties include the
direct provision of physical therapy services of home
care patients, as well as care coordination/management;
utilizes the professional health care process of assessment,
problem identification/planning, action/intervention
and clinical goal/outcome evaluation.
Candidates
must be a graduate of an American Physical Therapy
Association accredited Physical Therapy Program. Current
Oregon licensure or eligibility required. Two years
experience in an inpatient setting is required, with
home health and/or geriatric experience preferred.
